# Survey templates.

Templates for asking a group rather than an individual — team health, pulse checks, onboarding, retrospectives and coaching effectiveness.

[4Ls retrospective template The four Ls of a retrospective — liked, learned, lacked, longed for — what each is really asking, and how to run it so answers turn into changes. 8 questions](https://www.crewmojo.com/performance-management-templates/4ls-retrospective-survey) [Coaching effectiveness survey One question that tells you whether your managers are actually coaching, plus the follow-ups worth adding once you know. Take it and change it. 7 questions](https://www.crewmojo.com/performance-management-templates/coaching-effectiveness-survey) [Onboarding survey template Onboarding questions split across first week, first month and first quarter, because asking everything at day 90 gets you nothing you can fix. 14 questions](https://www.crewmojo.com/performance-management-templates/onboarding-survey) [Pulse check template Ten questions short enough to run every month — workload, support, growth and whether anything is in the way. Take it and change it. 10 questions](https://www.crewmojo.com/performance-management-templates/survey-template-pulse-check) [Team health check survey Fifteen statements that show whether a team trusts each other, argues productively, commits to decisions and holds each other to account. Take it and change it. 15 questions](https://www.crewmojo.com/performance-management-templates/team-health-check-survey)

## Choosing between them

- Decide what you will do with the answer before you ask the question. The fastest way to kill a survey is to run it and say nothing afterwards.
- Match the cadence to how fast the thing you are measuring can move. Pulse questions survive monthly; team health and coaching do not, and asking too often produces noise that looks like change.
- Protect anonymity properly on anything about a manager. Below about four responses people can work out who said what, and once that happens you stop getting honest answers from that team.
